I have tid bit of info on earpieces. I work with tech that allows a user to hear music or the phone through the inner ear, so the ear piece does not need to be inside the ear, just on the skull. The audio vibrations reverberate the inner ear – in my item the jaw, and sounds like you are wearing headphones.
I am in the bicycle industry and one of my products sales items was a bicycle helmet with this tech that attached to the helmet chin straps and vibrated the jaw this way. It was not a vibration you could feel, but it could be heard. This is not space science, but just common audio technology widely used.
Perhaps Joe Biden had something placed out of sight that his inner ear could pick up as hearing.
I do not rule out, however, that a very small device could be placed under the skin, or surgically so it could not be seen even with close examination.
